Health Management Services

Health and chronic condition management programs provide education, coaching and care coordination to help you manage ongoing conditions and support long‑term health.

Health management services are designed to help you make informed decisions about your health and better navigate the healthcare system. UofL's health management services may include condition management support, care coordination, health coaching and access to tools that help you understand treatment options, medications and preventive care. Through our health management services, you can receive support for managing chronic conditions, addressing health risks early and connecting with appropriate care when needed. These services complement our wellness and benefit programs by empowering you to take an active role in your health and promote long‑term well‑being for you and your family.

Health Advocacy Chronic Condition Support Program
Through the Chronic Condition Support Program, medically enrolled employees receive regular confidential phone sessions with a Nurse Coach who will help you manage your condition and improve your health and quality of life.
Livongo Diabetes Support Services
The program is offered at no cost to employees and dependents who have diabetes and are covered through one of the offered UofL health plans.
Advocacy Services
Health Advocate’s advocacy service helps you, your spouse, dependents, parents and parents-in-law navigate the healthcare system and supports you every step of the way, while Anthem’s Cancer Care navigators are ready to help manage your cancer care.
