Participating in a diabetes management program is important because it helps you control your blood sugar, prevent complications and support your overall health and well-being.

Eligible employees and dependents enrolled in a UofL medical plan with type 1 or type 2 diabetes can enroll in the Livongo diabetes management program. Receive a welcome kit including a blood glucose meter, test strips, lancing device, lancets and carrying case. Access Livongo’s secure website or app for personalized support and resources.

Who can join Livongo's diabetes management program:

The program is offered at no cost to employees and dependents who have diabetes and are covered through one of the offered UofL health plans.

What you get:

  • Connected Meter: Automatically uploads your blood glucose readings to your secure online account and provides real-time personalized tips.
  • Support from Coaches When You Need It: Communicate with a coach anytime about diabetes questions on nutrition or lifestyle changes.
  • Unlimited Strips at No Cost to You: When you are about to run out, we ship more supplies, right to your door.

How to enroll

Members complete a health profile: Required Information Gathered

  • Personal Info: Name, Date of Birth, Address, Email, Password, Company Registration Code (listed above).
  • Insurance Info: Health insurance provider, Group & Member ID, Express Scripts pharmacy ID to validate eligibility .
  • Health Profile: To better personalize the program and create a tailored experience from the start of the program for each member.
