University of Louisville Health Plan
The University of Louisville Health Plan (ULH) will keep your out-of-pocket medical expenses down when you use UofL providers. It provides you high coverage if you see certain ULH providers in the Anthem Blue Cross Blue Shield (Anthem) network, and provides a lower level of coverage when you see other providers who are in the Anthem network. Out-of-network coverage is not provided except for emergency care.

The plan offers three tiers of coverage:
- Tier 1 – UofL Health providers; lowest out of pocket costs
- Tier 2 – Anthem in-network providers; higher out of pocket costs
- Tier 3 – Out of network; coverage for emergency services only
Using UofL Providers:
- Very low cost when you using UofL Health (ULH) providers who are considered “high performing” by Anthem
- $0 deductible when using ULH providers
- Lower payroll deductions than other plans offered
- Inpatient stays, imaging and outpatient surgery will have copays instead of coinsurance, and patients will know amount of cost before receiving services A
- ULH primary doctor and specialist visits are free for you — no copay
- You will pay a copay for services like inpatient stays, imaging and outpatient surgery — since a copay is a fixed dollar amount, you will know your cost before receiving any service
Finding a ULH Provider:
- Go to anthem.com
- Select Find care (the green button) at the top right corner.
- Scroll down to the search bar labeled "Member ID number or prefix (first three values)".
- Enter ULQ (to find providers in EPO, PPO, PCA High, PCA low plans).
- Enter EUL (to find providers in ULH plan).
- Hit search.
- Enter the zip code of the provider and select Primary Care.
*All in-network providers should be seen.

Using non UofL Providers in the Anthem network:
- You will need to satisfy a deductible of $500 (per person) or $1,000 (per family) before coverage begins
- You will pay a copay for an in-network primary doctor or specialist visit
- You will pay coinsurance for services like inpatient stays, imaging and outpatient surgery, and these prices aren’t determined in advance
