University of Louisville provides benefit plans to meet the needs for you and your family members. If you are a new hire, you must enroll within 30 days of your date of hire. If you have astatus change, you have 30 days from date of event to modify your elections. Remember, the choices you make will remain in place through December 31st and you are not able to add or drop coverage until the next Open Enrollment period.
Dependent Verification Audit
The University of Louisville is conducting a Dependent Verification Audit for university employees and retirees who have dependents enrolled in the University of Louisville health, dental, vision and/or dependent life insurance. To maintain comprehensive and competitive benefits for our faculty and staff, the university must ensure that everyone covered under our plans is eligible for coverage. You may have already or will soon be receiving a letter which will give instructions on how to submit the supporting documentation needed to ensure continued coverage for your dependent(s). Employees will be contacted by a legitimate third party source (Aon Hewitt) who is conducting this audit, including Dependent Verification Center (mail), firstname.lastname@example.org (email) and Dependent Services (phone). Affordable Care Act - IRS Form 1095-C
As a result of the Patient Protection and Affordable Care Act, you may receive a new tax form known as the 1095-C. This form confirms your employer offered you affordable and essential health care coverage if you worked an annual average of 30 hours per week for the 2015 tax year.
The IRS deadline for employers to provide the Form 1095-C to employees is March 31, 2016. You do NOT need the Form 1095-C to file your personal tax return. Visit Frequently Asked Questions for more information regarding Form 1095-C.
Important Anthem Update
On, January 29, 2015, Anthem, Inc. (Anthem) was a victim of a highly-sophisticated cyber-attack. Information accessed may have included names, dates of birth, Social Security numbers, health care ID numbers, home addresses, email addresses, and employment information.
Identity Protection Services
Anthem has provided information on how to immediately enroll in identity protection services to all members regardless if you have been impacted. You do not need to wait until you receive a letter or email from Anthem to get started.
- Current and former Anthem members can visit AnthemFacts.com to learn more about credit monitoring and identity theft repair services provided by AllClear ID, a leading and trusted identity protection provider. All services are available at no cost to members for two years. After two years, members may continue AllClear ID services at a reduced monthly rate.
