Preferred Provider Organization Plan (PPO) with HRA
PPO Plan
- Allows you to go in or out of network with this plan, but you’ll pay less when you use Anthem providers.
- Comes with the ability to pay flat dollar copays for doctor’s office visits (and $0 if you obtain primary care from a UofL provider).
- Requires you to pay the full cost of medical services, before you meet the annual deductible. After you meet the annual deductible, the plan pays a percentage of the cost of most services until you reach the out-of-pocket maximum.
- Has its own prescription drug annual deductible and out-of-pocket maximum.
- Costs the most out of your paycheck, but less out of pocket when you need care.
- Embedded Deductible: Each individual only needs to meet their deductible and out-of-pocket maximum before the plan begins paying its share for that individual. Learn more about embedded deductibles by watching this video.
This plan comes with a Health Reimbursement Account. To make the most of your PPO Plan, it’s important to utilize your Health Reimbursement Arrangement (HRA). The HRA is designed to cover your full in-network deductible, helping you manage out-of-pocket medical costs more effectively.
Your HRA is funded by University of Louisville (UofL) contributions and administered through Optum Financial. You’ll receive a debit card preloaded from Optum with funds based on your coverage tier:
- Employee Only: $250
- Employee + Spouse/Qualified Adult: $500
- Employee + Children: $750
- Employee + Family: $750
These funds can be used exclusively for eligible medical and prescription expenses. When you need care, simply swipe your debit card.
