Members complete a health profile: Required Information Gathered

Personal Info: Name, Date of Birth, Address, Email, Password, Company Registration Code (listed above).

Insurance Info: Health insurance provider, Group & Member ID, Express Scripts pharmacy ID to validate eligibility .

Health Profile: To better personalize the program and create a tailored experience from the start of the program for each member.
