Clinical Compliance Requirements
Upper Division
Admitted upper division students are required to submit clinical compliance documents electronically by the appropriate cohort deadline. The School of Nursing uses Castlebranch for new students entering all School of Nursing pre-licensure programs. Castlebranch will manage all student documents, however it is the student's responsibility to maintain clinical compliance. Admitted upper division students will receive more information regarding how to complete their clinical compliance and purchase their compliance tracker in their upper division admission packet.
Bachelor of Science in Nursing program clinical compliance deadline: Spring term-December 10th. *Note this deadline is for the last current curriculum cohort of upper division. **Note students admitted to the revised upper division curriculum cohort will complete compliance during their first semester of nursing school. Materials are not allowed to expire during the term of enrollment. The term runs from the first day of class through the last day of final exams.
- CPR Certification:
Must complete either the American Heart Association: BLS for Healthcare Providers or American Red Cross: BLS for Healthcare Providers course. Certification cannot expire during a semester term. Student is responsible for renewal of certification for compliance. - HIPAA Training Courses:
Register for HIPAA course via the CITI platform (www.citiprogram.org). Upon completion, a printout of the completion page for "HIPAA Privacy-Students and Instructors" (ID:109374) must be submitted to the School of Nursing. Required prior to first semester of clinicals. Does not have to be renewed unless notified by the university.
Requirement fulfilled by submitting a copy of the Completion Page. - Bloodborne Pathogen Compliance:
Course offered online by the university on the Department of Environmental Health and Safety DEHS website. Provides a Certificate of Completion.
Required Yearly.
Requirement fulfilled by submitting a copy of the Certificate of Completion. - Nurse Student Liability Insurance:
Please apply early. It may take 4-6 weeks to receive your policy after you apply. Effective date of policy should be first day of semester. Nursing Student Coverage with limits of $1,000,000 to $3,000,000.
Requirement fulfilled by submitting a copy of the policy cover page that has been issued with an effective date and coverage limits. An application is NOT proof of coverage. - Nurse Aide Training:
The NAT requirement can be met in one of four ways:
- Successful completion of NURS 221: Principles of Basic Patient Care with a C or higher.
- Submission of documentation of 100 hours of direct health patient care work experience within the past 12 months OR 1000 hours of employment within 10 years of upper division clinical compliance deadline. If you are currently working in health care, do not assume your work assistance will meet this requirement. Please check the NAT FAQ document. - Documentation of completion of a Medicaid approved State Registered Nursing Aide Program. Students are not required to take the SRNA certificate exam.
- Active certification as a State Registered Nursing Aide (SNRA). Students meeting this requirement must submit proof of SNRA certification from the Nurse Aide Registry.
- Immunization Compliance:
This must be completed at the Campus Health Services office. Check with the immunization specialist at 502-852-2708 to confirm you are in compliance. Requirement fulfilled through notification from Campus Health Services to the School of Nursing. No document submission required by student. - Drug Screening:
Please go to the following website for instructions on how to register for the test: https://portal.castlebranch.com/IP65. Use the same account that you used when you established your background check. If you don’t know your account information, contact the CBC Customer Service Department at (888) 666-7788 ext. 7194. Your package code for Louisville Campus UofL is Ni53dt.Required for first semester clinicals. Initial drug test must be submitted 6 months or less prior to the first semester clinical compliance date. Thereafter drug tests must be submitted annually.
Requirement fulfilled upon the SON receipt of test results from the Castlebranch company. All results will be kept confidential. No document submission required by student.
The testing site closest to you can be found on the Castlebranch.com website. Please keep your login and password in case it is needed for future use. Students will be required to pay for the testing.
You will receive the results of your drug screen. However, if you are unable to provide evidence of a negative drug screen, you will not be allowed to attend clinical and thus must withdraw from all clinical courses. Please note that Kentucky Revised Statute (KRS) 314.031 has mandatory reporting requirements for licensed nurses that the SON must follow. - Clinical Compliance Student Consent and Release Form
Requirements fulfilled upon SON receipt of signed form. - COVID Verification:
One of the following is required: 1) Proof of COVID-19 vaccination(s) documented on the CDC card. NOTE: the CDC COVID-19 Vaccination Record is the only acceptable vaccine documentation. Vaccine documentation must include the vaccine manufacturer. Provide COVID vaccine card (submit even if you have only received 1st of a 2-dose series, must re-submit updated card upon 2nd vaccination). OR 2) Documentation of medical or religious exemption approved by UofL Campus Health. Clinical agencies do not accept conscientious objection exemptions. Exemption Form available on Campus Health webpage. Instructions for uploading the COVID immunization record (outside KY only) or Request for Exemption form to Campus Health are listed below.
All students must also submit appropriate COVID vaccine documentation to the School of Nursing (see link below based upon enrollment date).
Directions for submitting out-of-state COVID vaccine or exemption form to Campus Health:
- Proceed to Campus Health patient portal: https://louisvilleportal.pointnclick.com
- Log into the site using ULINK@louisville.edu and ULINK password.
- Confirmation page will open, enter your date of birth.
- Patient portal will open.
- Click on Immunization Upload at the bottom of the menu.
- Click on Add Immunization Record button to upload vaccine records from outside Kentucky (if vaccinated in KY, you do not need to submit.) OR Click on Downloadable Forms on the left side of the page.
- Download COVID Vaccine Religious Exemption Form OR Medical Exemption Form
- Complete your exemption form.
- On the Immunization Upload page, click the Add Immunization Form to upload your exemption form.
- Once finished uploading your form, click SAVE.
