INSTRUCTIONS ON HOW TO SUBMIT YOUR CLINICAL COMPLIANCE DOCUMENTS ELECTRONICALLY

Go to the Student Clinical Compliance OnBase upload link

Start by entering your UofL seven digit student ID and your name as listed on your student records.  The School of Nursing must use your UofL student email address for all correspondence related to your clinical compliance status.  

Please use either Mozilla Firefox or Google Chrome browser and format your uploaded documents in either a Word document, PDF, or a jpg image.  If you receive an upload failed, please open a different browser and try again.


CPR Certification
Enter the expiration date listed on the front of your American Heart Association BLS for Healthcare Providers card.  Upload the front of the card showing your full name and the expiration date.  You do not have to upload the back of card.

HIPAA Training Courses
Upload the Completion Page from the HIPAA training.  Be sure your full name and date of completion show on the uploaded document.  No dates are required to be entered when you submit this document.

Bloodborne Pathogen Compliance
Enter the expiration date listed on the Certificate of Completion. The date is typically found in the lower right hand corner.  Upload the certificate showing your full name and expiration date.

Professional Liability Insurance
Enter the expiration date listed on your policy.  Upload the Policy Cover page that shows the effective dates of coverage and the coverage limits.  Do not submit the application or the full policy.

Clinical Compliance Student Consent and Release Form
Form available on the nursing website.  Print out to complete.  Upload completed form. No dates are required to be entered when you submit the document.

Registered Nursing License
Enter the expiration dated listed on your license.   Upload the verification from the KBN or state registry where you are licensed.

COVID Verification
Appropriate documentation must be submitted to Campus Health and School of Nursing
