Who needs to complete the ADF?

Effective October 1, 2017, the Attestation and Disclosure form must be completed, at least annually by:

  • All University of Louisville Employees.
  • All individuals participating in research under the auspices of U of L, regardless of compensation. This includes all individuals with a research appointment.

External investigators (consultants/collaborators/etc.) named on Public Health Service (PHS) and National Science Foundation proposals must have statements submitted to the COIC Office prior to proposal submission if they are not affiliated with organizations that are federally assured. Statements are available via the form for proposals. Please see the above tip-sheet regarding the investigator descriptions.

Additional requirements are required upon award for all external investigators (consultants/collaborators/etc.) on projects with FCOI PHS requirements. Contact the COIC Office for assistance.

How do I complete the ADF requirement?

The Attestation and Disclosure Form (ADF) is available upon logging in to the iRIS system. The instructions for completing the ADF are available for download. Please follow the instructions for the successful completion of your ADF.

Travel Disclosure Form

The Travel Disclosure Form is used ONLY to report any paid or reimbursed travel from an external entity (not UofL) that exceeds $5,000 annually from October 1 - September 30.

This form will not fulfill your annual Attestation and Disclosure Form (ADF) requirement.

How do I complete the COI training course in CITI?

Effective October 1, 2015, the ADF now also contains the Conflict of Interest training element; however, the Conflict of Interest training Course will remain available on the CITI training platform.

Any individual that has completed the ADF on or after October 1, 2015, as a covered individual involved in research, has completed the COI training requirement, but can access the additional COI training on CITI.
