Appointments - Patient Referral

To schedule an appointment, call 502-588-4710, and ask to make an appointment in the Pulmonary ILD Clinic.

Please complete the Medical History and Symptom Questionnaire prior to your visit with us.

What should I bring to my appointment?(if available)

  • Insurance Card
  • Breathing Test Reports
  • Blood test results
  • List of medicines
  • Lung Biopsy Reports
  • X-ray or CT Scan (please hand-carry actual films or CD)
  • Other Medical records

What can I expect at my first visit?

The UofL ILD Clinic doctor will review your medical history, X-ray films, and perform a physical examination. You will also have your oxygen level checked while sitting and again while walking. Other tests may be necessary and can possibly be done the same day as your visit. These include:

  • Chest X-Ray or Chest CT Scan
  • Breathing Test
  • Blood Tests

What do I need to know if I am scheduled for a breathing test?

Eat only a light meal, avoid caffeine drinks and DO NOT SMOKE for at least 4 - 6 hours. Please remember to wear comfortable clothing and walking shoes.

Patient Referrals

Referring healthcare providers can easily request patient consults (new and second opinion), obtain patient consultation notes and reports, request prescription information, obtain information about clinical trials and inquire about a number of other services provided by the UofL Interstitial Lung Disease Program.

  • Progress Notes that include the medical history and physical examinations
  • Laboratory blood test results
  • Pulmonary Function Test Reports
  • Any X-ray or CT Scan reports.
  • Tissue biopsy report
  • Medications or other prescribed therapies
