Fellowship Application & Contact
Thank you for your interest in the University of Louisville Primary Care Sports Medicine Fellowship Program. Applicants should have a demonstrated interest in sports medicine (previous experience, event coverage during residency, elective rotations in sports medicine, etc.).
All applicants must have satisfactorily completed an accredited residency in Family Medicine, Internal Medicine, Med/Peds, or PM&R, and must be:
- Board eligible or board certified in that field
- Eligible for an unrestricted Kentucky license
- Eligible for an unrestricted DEA registration
The University of Louisville Primary Care Sports Medicine Fellowship participates in the National Residency Match Program (NRMP). Applicants must register and participate in that process. For dates and registration please read more at www.nrmp.org.
How to Apply
Applications will only be accepted through ERAS. All applicants must apply through ERAS for consideration for an interview. Our deadline for receipt of your application is October 1st. Interviews are by invitation.
Documents required in Application Package:
- Complete ERAS application with photo
- Three letters of recommendation, including one from your residency program director
- A personal statement that outlines your personal experiences in sports medicine, your goals for fellowship, and your areas of interest
- Curriculum vitae
- USMLE or COMLEX transcripts (provided by National Boards)
- Most recent In-Training Exam Results
- ECFMG certificate if applicable
- Medical School Diploma and transcript
- Current State Medical License
