Forms

Service Request
Students who have been working with the Disability Resource Center should submit a Service Request Form to their coordinator as soon as possible after registering for their classes each semester. Priority deadlines will be set each semester, so contact your coordinator early. -
Schedule Exams
After requesting an accommodation letter and providing it to their instructor, students who will test at the DRC may schedule their exams online. -
Alt Text Request
Students who have been approved for an accommodation of books in alternate format should submit their book requests using this form as soon as possible after registering for classes. -
Housing Accommodations
Students may request disability-related accommodations in Campus Housing and find related procedures here. -
Emotional Support Animal Request
Students may request allowance to bring an emotional support animal into their Campus Housing residence as an accommodation for a disability here.
