Candidates completing their first teaching certificate will receive a Statement of Eligibility from the Kentucky Education Professional Standards Board (EPSB). This is issued free of charge. To apply for the Statement of Eligibility, please complete a CA-1 form.

The documents noted above must be submitted to the Education Advising and Student Services Office (room 253 of the Education Building). The certification officer will complete and sign the last page of the CA-1 form, and send the documents to the state certification office. The Statement of Eligibility (initial teaching license) will be mailed directly to the candidate from the state certification office.
Candidates seeking a rank change or additional certification must complete a CA-1 form.

The new certificate/rank change will be mailed directly to the candidate from the state certification office. Fees associated with applying for a rank change or additional certification should be paid directly to the state certification office. Please go to www.kyepsb.net and click on "Pay Certification Fee" for further information and on-line payment options.
To apply for a 15-hour (towards rank II or rank I) pay increase, please complete the 15-Hour form to the Education Advising and Student Services Office. (Please do not submit this request until the grades for all 15 hours have posted)
Once the 15 hours is confirmed, it will be verified to the school district and an e-mail notification will be sent to the applicant. Please be aware that not all school districts issue 15-hour pay increases. If you are unsure, please contact the Human Resources Office of your district.
