15 Hour Verification Request

For those who have completed 15 hours towards a planned Rank I or II program and qualify for a pay increase. There must be a planned program signed by a faculty adviser on file in the Education Advising and Student Services department of the College of Education for this to be processed.

Please complete this form if you are currently employed with a local school system and have completed (all coursework must be done and graded) 15 hours towards either your Rank I or Rank II program.
