Materials required for promotion and/or tenure are:

• Current CV in the U of L format
• Chair's letter of support, Department PAT review (if applicable), department ballot
• Copy of acceptance letters for articles listed as "in press" or "accepted" on CV
• One set of reprints of most significant publications; one copy of all reprints since last appointment or promotion
• Four extramural evaluation letters
• Copies of annual work plan for period of review
• Copies of annual performance (SIBUP) reviews for period of review
• Peer summative teaching evaluation form
• Teaching evaluations from trainees during period of review
• Copy of mid-tenure review for faculty on tenure track
• Personal statement
• Copies of grant awards - UBM-13A award summary
