|Academic Training for J-1 Students
“Academic Training,” is the name used by the Department of State for certain types of study-related employment for students in J status. It describes the conditions that have to be met in order to qualify for “Academic Training” and how to apply.
|Change of Educational Level (F-1) or Matriculation (J-1) Request Form
An F-1 student who is requesting to change from one educational level to another or a J-1 student requesting a matriculation, must complete this form.
|Change of Status Request Form
If applying to change your nonimmigrant status while in the U.S., submit this request form to obtain an Initial Attendance-Change of Status Form I-20.
|Check-In Procedure for New International Students & Scholars
As soon as you arrive at UofL, you must submit the following documents: Visa, I-94, and current local address to the ISSS service account.
|Dependent Request Form I-20/DS-2019
F-1 or J-1 students requesting that their spouse or children join them should complete this form.
|Letter Request Form
Students who need a letter for good standing, letter of support, driver's license or a letter inviting family members, you may submit this form.
|Meal Plan Waiver Form
Students requesting a waiver from a meal plan must complete the form and meet the criteria.
|OPT: Personal and Employer Information
When you receive employment on your OPT, you must complete this form. Submit this form every time you update your address, change employers, or have multiple employers. You will be issued a new Form I-20 with every new employment. If you are on STEM OPT, you may use this form to submit your validation report.
|Program Extension Request Form
If you are not able to finish your degree program by the end date noted on your Form I-20 (F-1) or DS-2019 (J-1), you can request an extension of your program. The length of time allowed for an extension is based on the recommendation of your academic advisor.
Please complete and sign the release of information statement and give this to a Designated School Official at the school you are currently attending.
If you plan to transfer from the University of Louisville to another institution in the US, you must notify the International Center of your intent to transfer. We will then release your record electronically to the new school. Upon receipt of this form and an admittance letter from your new school, we will transfer your information, thereby granting the new school access to your record.
