Participation and Release Form
To protect the University’s people and property, individuals participating in events or activities outside their normal scope must sign a Participation and Release form. Departments should provide a supplemental addendum outlining potential risks. Access the form below.
Commercial Casualty Insurance Program

Workers' Compensation
Injured on the job? Employees can see any physician for treatment and must notify their supervisor to complete a Workers' Compensation report. For immediate attention, visit any emergency facility. For more info, log in to our Workers' Compensation page.
Youth Protection Program (YPP)

Who We Are
The Department of Enterprise Risk and Insurance (ERI) aims to minimize risks to individuals, property, and the business by safeguarding the University community. ERI oversees the University of Louisville’s Enterprise Risk program and manages various insurance policies, including general, property, auto, and professional liability, as well as workers' compensation. ERI also provides policy oversight for university programs involving minors. By collaborating with all departments, ERI evaluates and mitigates potential risks while promoting a culture of proactive risk management across the University.
