Filing an Informal Report with the Title IX Office

Title IX distinguishes between Informal Reports and Formal Complaints. An Informal Report may be submitted by anyone to notify the university that sexual misconduct may have occurred. A Formal Complaint is a document signed by a complainant requesting an official investigation and resolution process. 

Upon receiving an Informal Report, the Title IX Office will:

  • keep the reported information as private as possible, while staying within the law and university policy;
  • work with the complainant to meet their support needs; and
  • discuss options for moving forward under university policy.

Making an Informal Report to the university does not require a complainant to file a Formal Complaint.

Anyone can file an Informal Report of sexual misconduct, including sexual harassment, with the Title IX Office or the University of Louisville Police Department. If you suspect that you or someone you know has been impacted by sexual misconduct, let us help.

Complete the Title IX Report Form. Once your report is received, the Title IX Office will reach out to provide supportive measures and guide you through your formal reporting options.

File a report with the UofL Police Department

The UofL Police Department investigates criminal violations. In an emergency, call 911. Members of the university who believe they have been subjected to sexual misconduct or need more immediate assistance are encouraged to report to law enforcement.
