Enterprise Security Forms

  1. Encryption Exception Form

    For individuals or Tier1s applying for an exemption on required encryption a university device or machine.

  2. Firewall Security Modification

    For requesting new firewall rules, changes to existing firewall rules or deletion of current firewall rules. Please note: All firewall rule requests will be carefully reviewed before a decision on approval in order to maintain university-wide security.

  3. Custom Firewall Creation

    For requesting a custom firewall setup for computers needing protection. Please note: There is a one-time set up charge of $390 and an annual recurring charge (CAR) of $182. Contact information of two users (or more) required on form for all subsequent request changes.

  4. System / Process Authentication

    For employees to begin the request process for allowing their system and/or process to authenticate in the central identity management system against one of the University's directories.

  5. Certification Request

    For requesting a digital certificate (sometimes called an X.509 certificate) in order to protect an application communicating over the network. The certificate is used in the Transport Layer Security (TLS) protocol to validate the certificate owner, the University and encrypt the data communicated over the network.

  6. UofL Technology / Cyber Incident Response

    Information Technology responds to and investigates misuse or abuse of University of Louisville technology resources, including computer and network security breaches and unauthorized disclosure or modification of institutional or personal information. This form begins the reporting process.

  7. Open Records Request

    For those with a legal purpose to request documents from the University.

  8. Copyright Violation

    For individual whose access to UofL systems has been suspended due to being accused or identified by a copyright holder as in violation of copyright law. Please note: Read the form carefully. Failure to follow all instructions will delay the process of review and subsequent actions.

  9. Mass Email Send Request

    For request to send an email message to more than 500 recipients. Without approval, the message will fail to be delivered to the intended audience.

Account Management Forms

  1. Sponsorship Request for Computer Accounts

    For requesting two types of computer accounts: Individuals who are directly employed or affiliated with UofL such as Student Assistants, non-enrolled students or Board of Trustee members; Individuals who are external to the University but have a relationship with a department or unit for business or research purposes.

  2. OnBase Account Request

    For access in order to create an OnBase account.

  3. Computer Account UserID Change Request

    If your name has changed, this form submits a request for changing a current UofL UserID.

  4. Distribution Group

    System wide lists can consist only of Exchange email users on the university system. Distribution groups must be created at the system level and requires the completion of this form.

  5. Service Accounts

    For requesting a new service email account, change the ownership of a current service account or to close a current service account.

  6. Shared Resources (Exchange) Request

    For requesting permission to link accounts or granting access rights for sharing resources within Exchange.

  7. Create or Modify Shared Directory

    For units or departments that do not have Tier 1 technology personnel. Request to grant or modify access to a department shared folder.

  8. University Reports Account (Campus Solutions)

    Access to the Campus Solutions University Report system is limited but requests are taken with this form. All requests are reviewed, although some may be denied.

  9. University Reports Account for Human Resources

    Requests for Human Resources PeopleSoft access must be sent to Becky Letcher using University email. Include name, UserID, department, job title, department ID(s) to which access is needed and justification for access. All requests are reviewed, although some may be denied.

  10. Orion Account for Human Resources

    Requests for Human Resources PeopleSoft access must be sent to Becky Letcher using University email. Include name, UserID, department, job title, department ID(s) to which access is needed and justification for access. All requests are reviewed, although some may be denied.

  11. PeopleSoft Account for Campus Solutions

    Access to PeopleSoft for Campus Solutions is limited to staff that who need to view and/or update student records or course information on a regular basis to perform their job functions. Requests are taken with this form.

  12. PeopleSoft Financials Account

    Requests for PeopleSoft Financials access must be sent to Dave Baugh using University email. Include name, UserID, department, job title, department ID(s) to which access is needed and justification for access. All requests are reviewed, although because access is limited to staff with a need to view finance information some may be denied.

  13. Tier 1 Designation request

    For granting or removing Tier I employee access to the following: Tier 1 professional email listserve (TierTalk), Tier 1 resource list on support site and Admin access to the departmental Active Directory organizational unit (OU).

  14. Create or Modify Department Scratch Folder

    Departmental Scratch Folders are a shared workspace for co-workers on an IT server from which all files are deleted every night. These are located on the "I" drive under the scratch directory (i.e. I:\scratch\name). Tier 1s and UBMs can request a shared scratch folders.

  15. Home Directory Within Active Directory Request

    For requesting a home directory within Active Directory.
