  1. System Center Endpoint Protection Exception
    This form will be used to substantiate the need to obtain University antivirus software, System Center Endpoint Protection. Formerly known as Forefront Endpoint Protection, Forefront Client Security and Client Protection. This should only include machines that cannot join the University's Active Directory Domain. This exception does not include home machines.
  2. Firewall Security Modification
    Please use this form to request new firewall rules, changes to existing firewall rules, or to delete current firewall rules. All firewall rule requests will be carefully reviewed before granting approval. While firewall rules are sometimes necessary their widespread use degrades security for the entire campus community. Please note: If after the firewall rules have been applied the computers are identified as insecure, the firewall rules can be deleted without notice.
  3. Custom Firewall Creation
    Please use this form to start a Custom firewall setup for computers that you want to protect. There is a one-time set up charge of $390 and an annual recurring charge (CAR) of $182. At least two users are required for contacts as they will be the only people that can request changes. If you want more than two contacts list them in the last field on the form.
  4. Request a Server Exception
    The University of Louisville maintains an enterprise class secured data center for the housing of university servers. Any server not maintained in the university data center must be approved via ISO PS004 Policy Exception Management Process.
  5. System / Process Authentication Request
    Request system/process authentication against the Universities central identity management system. This form is for employees to start the request process that will allow their system and/or process to authenticate against one of the University's directories.
  6. Certificate Request
    Use this form to request an SSL certificate.
  7. UofL Incident Response Form
    Information Technology, Enterprise Security responds to and investigates incidents related to misuse or abuse of University of Louisville technology resources.  This includes computer and network security breaches and unauthorized disclosure or modification of institutional or personal information.
  8. Server Checklist
    If you need a server for departmental use complete this form.
  9. Server Patch Exception
    Use this form when a server meets the criteria in the server maintenance policy for an exception.
  10. Open Records Request
    If you have a legal purpose to have documents from the University have your lawyer fill out this request.
  11. Copyright Violation
    This form is for anyone who has been identified by a copyright holder as violating their copyrights and your access has been suspended. Failure to follow any and all instructions will delay the return of your Internet access. It is in your best interest to read and understand everything below.

Account Management Forms

  1. Sponsorship Request for Computer Accounts
    This electronic form is used to request computer accounts for two types of individuals. They are: Individuals who are directly affiliated with the University of Louisville such as Student Assistants, non-enrolled students and board of trustee members who may or may not be listed in the University's database. Individuals who are external to the University but have a relationship with a University unit for business or research purposes.
  2. Request for OnBase Accounts
    If you need access to OnBase request an account.
  3. Computer Account UserID Change Request
    If your name has changed, use this form to reflect the change in your UserID
  4. Distribution Group Request
    System wide distribution groups must be created at the system level and requires the completion of this form. System wide lists can consist only of Exchange users on the university system.
  5. Service Account Request
    This electronic form is used to request, a new Service account, change the ownership or close an Service account
  6. Exchange Shared Resources Request
    Due to system constraints, the resource name cannot contain any spaces. Spaces will be replaced with underscores for readability.
  7. Exchange Service Account Permissions
    This form is only needed for Service Accounts that migrated from Groupwise. Use this form to identify users who need full access to everything in your Service Account. The form must be completed by the owner of the Service Account
  8. Exchange E-Mail Account for Student Assistants
    Use this form to submit a request to open an Exchange E-Mail account for student workers/assistants. This will enable them to use a separate email account for work-related emails.
  9. Create/Modify Shared Directory
    This form should only be submitted if your department does not have a Tier I. If your department does have a Tier I, please see them to grant access to a department shared folder.
  10. University Reports Account for Campus Solutions
    Access to the Campus Solutions University Report system is limited to staff with a need to view the information. All requests are reviewed and some may be denied. You can track your request using HEAT once the initial review has been performed.
  11. University Reports Account for Human Resources
    This form will only be accepted if submitted by Becky Letcher.  Requests for Human Resources PeopleSoft access should be sent to Becky Letcher using University email, including name, userid, department, job title, department id(s) to which access is needed and reason access is needed. Access to the Human Resources PeopleSoft access is limited to staff with a need to view the information. All requests are reviewed and some may be denied. You can track your request using HEAT once the initial review has been performed and necessary approvals have been obtained.
  12. Orion Account for Human Resources
    This form will only be accepted if submitted by Becky Letcher.  Requests for Human Resources PeopleSoft access should be sent to Becky Letcher using University email, including name, userid, department, job title, department id(s) to which access is needed and reason access is needed. Access to the Human Resources PeopleSoft access is limited to staff with a need to view the information. All requests are reviewed and some may be denied. You can track your request using HEAT once the initial review has been performed and necessary approvals have been obtained.
  13. PeopleSoft Account for Campus Solutions
    Access to PeopleSoft for Campus Solutions is limited to staff who need to view and/or update student records or course information on a regular basis to perform their job functions.
  14. PeopleSoft Financials Account
    This form will only be accepted if submitted by Dave Baugh. Requests for Financials access should be sent to Dave Baugh using University email, including name, userid, department, job title and reason access is needed. Access to the Financial system is limited to staff with a need to view the information. All requests are reviewed and some may be denied. You can track your request using HEAT once the initial review has been performed.
  15. Tier 1 Designation request
    Submitting this form will grant or remove Tier I employee access to the following: The U of L Tier I professional e-mail list, TierTalk; Entry into the Tier I resources list located on the Two-Tier Technology Support Site or Administrative access to the departmental Active Directory organizational unit (OU)
  16. Create/Modify Departmental Scratch Folder
    A Departmental Scratch Folder is departmental workspace for you and your co-workers on an IT server from which all files are deleted every night. It will be located on the "I" drive under the scratch directory (i.e. I:\scratch\name). If you are the Tier I or UBM for your department you may request a shared scratch folder for your department.
  17. Request a Home Directory in Active Directory
    This form is used to request a home directory in Active Directory (effective March 19, 2012) for university employees.
  18. Cardmail Account Closure Request
    Use this form to submit a request to close your Cardmail E-Mail account if you are no longer enrolled in the university. The account will automatically close after a 2-year period of inactivity. Only use this form if you wish to have the account closed before the 2-year deadline. Any e-mail that is currently in the account at the time of closure will be lost.
  19. Large File Transfer Account Request
    Use this form to request an account to be made for transferring files larger than 20MB to external or internal individuals.
