Data Request Form

Currently all data requests require up to 7-10 business days to process.

Effective July 2020, requests for identifiable, protected class data will need to be accompanied by a business reason (please provide your reasoning as a separate attachment to your data request).  Your request and business reason will be forwarded to the Vice Provost for IR, Effectiveness, and Analytics as well as the Office of Diversity for approval.  Please anticipate additional time needed to complete these requests.  U.S. federal law recognizes the following nine protected classes: sex, race, age, disability, color, creed, national origin, religion, or genetic information (added in 2008)

To expedite your request, please include and/or attach relevant instructions in the space provided below. Questions about your data request should be directed to Shari Barrow, Project and Logistics Coordinator, at uoflirp@louisville.edu.
