Travel Card - Lost or Stolen

If you are submitting this Lost or Stolen form during normal business hours, the Travel area will handle requesting a replacement card. If outside normal work hours, please contact PNC Bank at 1-800-685-4039 to immediately deactivate the card and request a new one be sent to the attention of the Travel Office at the address PNC has on file.
