Waiver for HSC Required Influenza or COVID Vaccine

To submit a medical or religious waiver for mandatory flu vaccine please log into the patient portal at https://louisvilleportal.pointnclick.com and log in with your Ulink@louisville.edu email and associated password.  Once logged in click on DOWNLOADABLE forms and select the appropriate waiver to download and complete.   Once completed, return to the patient portal, open the downloadable forms section and upload your waiver under the appropriate form.
