How to Submit Records


All required documentation must be submitted to Campus Health no later than your scheduled orientation date.

Submission of vaccine records is a requirement for all first time freshmen. Failure to submit records, or incomplete records, will prevent you from registering for classes in the Spring semester.

How to submit records:


Hand Deliver: Bring to scheduled orientation and submit to Campus Health table during lunch session on Day 1 

Once records have been received Campus Health will verify all immunizations. Upon verification you will receive an email notifying you of any missing immunizations or notice of compliance.
