AWP Information

1. AWP Form: The AWP form is found on the A&S Faculty Affairs webpage. To find it follow this link then scroll to the bottom of the page and click on the AWP Form link. 

2. In order to complete the AWP Form you will need to use Adobe. Click on the link to download Adobe Reader if needed. *All UofL employees now have access to Adobe. To learn more go to https://louisville.edu/its/adobe-support.
