Campus Health Services Travel Clinic* offers:
- Comprehensive pre-travel consultation
- Travel and routine immunizations
- Malaria prevention
- Itinerary-based travel health counseling
- Completion of health clearance forms
* Clinic registered with International Society of Travel Medicine (ISTM)
How to schedule a travel visit:
- Complete Foreign Travel Questionnaire
- Send immunization records to Campus Health Services (Medical Records Release Forms)
- Call Cardinal Station 502-852-6479 or HSC 502-852-6446 for an appointment
PLEASE SCHEDULE AT LEAST 4 - 6 WEEKS BEFORE TRAVEL TO ALLOW ENOUGH TIME FOR YOUR IMMUNIZATIONS TO BEGIN PROTECTING YOU.
