How can I become a patient?
Call the dental school at (502) 852-5096 and press 2 for new patient appointments. You can complete an application and send it to Patient Services at 501 South Preston Street, Louisville, KY 40292. We can also mail one to you. The University of Louisville School of Dentistry is open 8:00 a.m. - 5:00 p.m.
What should I bring to my first appointment?
We require a photo ID, a $40.00 deposit (cash, debit card, or credit card), your dental insurance card if applicable and a list of your current medications.
What can I expect at the new patient screening examination appointment?
Your first appointment will consist of your health history, a comprehensive exam will be initiated and x-rays will be taken to determine what treatment you require.
What happens after I'm accepted for treatment?
You will be assigned to a student doctor either at your first appointment or within one week of your first appointment. By your third visit, a treatment plan will be presented to you with the estimated length of time for your treatment and the cost of your treatment.
How long will it take to complete my treatment?
It depends on the individual needs of each patient.
Who will provide my dental care?
Dedicated dental students, dental hygiene students and dental residents provide the majority of treatment while supervised and evaluated by our highly trained faculty.
What will my treatment cost?
The cost of treatment varies depending on your treatment plan once your consultation is completed.
How do I pay for my dental care?
The School accepts both patients with and without dental insurance. Payment plans can be arranged if patients qualify and Medicaid/Medicare/KCHIP is accepted. Please see Fees and Insurance for more information.
Is childcare provided?
Children must be accompanied by an adult at all times. The School does not provide childcare. If your children are left unattended, your appointment will be cancelled.
What if I need to miss an appointment?
The School allows patients to miss three scheduled appointments. We require 48 hours of notice if a patient has to cancel an appointment. If you cancel on the same day as your scheduled appointment, it would be considered a "no show" appointment. After three missed appointments, you may be dismissed from the program.
Can I be treated by a faculty dentist?
The School offers a Faculty Practice Clinic.
Can I receive specialty care without going through the screening process?

How much can I expect to save?
Generally, we save patients 30-50% of the cost of private practice.
Are there any disadvantages to coming to the school?
We offer the same quality dental care in a learning facility. This can result an appointments taking longer than they do in private practice.
How long does it take to be seen, once I've mailed my application back?
Once the application has been received and processed by Patient Services, there is generally a 7-10 day waiting period due to the amount of applications received. There are times that it takes longer due the academic calendar and the students being off, i.e., holidays and summer break.
