Avoid these common errors

Please be sure to read and complete forms properly before submitting. The following items should get special attention:

• Did you include your email address (UofL address preferred)?
• Did you fill in courses - with course numbers - correctly?
• Did you use correct terms, for example: Spring 2005?
• Is this a first petition or an appeal?
• Did you completely fill out the form?
• Did you sign and date where indicated (PDF or paper forms)?
• Did you remember to obtain your instructor's remarks, if needed?
