Program Requirements & Forms

Understanding the Program Forms

Program forms are available for all of the degrees offered in the Department of Fine Arts: the B.A. in Art History, the B.A. in Studio Art, and the B.F.A. programs in 2-D Studios, 3-D Studios, Interior Design, and Graphic Design.

Please do not hesitate to contact your advisor with any questions about requirements you do not understand. Not doing so could cause you to take courses you do not need and unnecessarily extend your stay at the University.

The General Education Requirements course list is available on the Office of the Provost website at: http://louisville.edu/provost/ger

The WR Requirements course list is available of the College of Arts & Sciences Advising website at: http://louisville.edu/artsandsciences/advising/mjrs/wr-courses

Need a program form?
Blank program forms are available in the Fine Arts Office. The Fine Arts office also keeps copies of the official Program sheet that is created at the time you are accepted as a major. If you would like a copy of your official program sheet, please email Theresa Berbet.
