University of Louisville provides benefit plans to meet the needs for you and your family members. If you are a new hire, you must enroll within 30 days of your date of hire. If you have a qualifying event status change, you have 30 days from date of event to modify your elections. Remember, the choices you make will remain in place through December 31st and you are not able to add or drop coverage until the next Open Enrollment period.

Benefits Open Enrollment for faculty and staff is October 16-27, 2017.  Elections made during this time will be effective January 1, 2018.

New Benefits Self-Service

Benefits enrollment is going online!  All new employees and current employees who have qualifying events throughout the year can now make their benefit elections online via ULink. Life Insurance beneficiaries can also be updated using Benefits Self-Service. Learn more about the new Benefits Self-Service options.

Affordable Care Act - IRS Form 1095-C

As a result of the Patient Protection and Affordable Care Act, you may receive a tax form known as the 1095-C. This form confirms your employer offered you affordable and essential health care coverage if you worked an annual average of 30 hours per week for the 2016 tax year.

The IRS deadline for employers to provide the Form 1095-C to employees is March 1, 2017.  Visit Tax Form 1095-C Frequently Asked Questions for more information regarding Form 1095-C.

Retirement 403(b) and 457(b) Beneficiary Designations

Our Retirement Investment Providers, Fidelity Investments and TIAA, will soon be reaching out to all University of Louisville 403(b) and 457(b) plan participants asking you to take a few minutes to review the beneficiaries on your account to ensure that your vested retirement account balance will be distributed according to your wishes.
