Student Complaints
Students are encouraged to provide feedback — including complaints — that helps to make ULSD a better environment for individuals and the entire school community. While providing feedback directly to a faculty member, academic area, or administrative unit is welcome, students may also use the links on this page to submit an online complaint directly to the School of Dentistry Dean's Office and/or University of Louisville Dean of Students Office.
If you need to report an urgent matter related to health or safety, please do not use these forms. Call 9-1-1, contact University of Louisville Police, or go to the Dean's Office (School of Dentistry Suite 240) right away.
