Report Flood Damage Instruction

EMERGENCY CLAIM

 

  1. Risk Management approval is required before discarding any damaged goods. 
  2. Submit all flood related damage expenses on the Report Damages Form (xls) :
    1. Time spent by all employee
      1. Hourly wages – straight time and overtime noted individually
      2. Salary for professional and administrative staff for time spent planning and collecting data for claim application
    2. Contracts for services and services performed
    3. Equipment rented
    4. Vehicles used, time running idle and mileage
    5. Equipment replacement expense
  3. Submit all Report Damage Forms to University Risk Management by email to RISKMGMT@louisville.edu or fax 852-0740.
