Report Flood Damage Instruction
EMERGENCY CLAIM
- Risk Management approval is required before discarding any damaged goods.
- Submit all flood related damage expenses on the Report Damages Form (xls) :
- Time spent by all employee
- Hourly wages – straight time and overtime noted individually
- Salary for professional and administrative staff for time spent planning and collecting data for claim application
- Contracts for services and services performed
- Equipment rented
- Vehicles used, time running idle and mileage
- Equipment replacement expense
- Submit all Report Damage Forms to University Risk Management by email to RISKMGMT@louisville.edu or fax 852-0740.
