Bringing Health Equity to the Underserved Communities of Smoketown, Shelby Park and Surrounding Areas
The Harambee Nursing Center exists to improve the health and quality of life for area residents. We strive to become a dynamic, creative, trusted, safe and sustainable exemplar of holistic health care that heals and nourishes the body, mind, and spirit of the underserved, urban residents of Smoketown, Shelby Park and Phoenix Hill.
Harambee is an African tribal term that means “a community coming together to better society.” The partners’ desire to improve the quality of life of this community is the driving force that established, and continues to grow, this vital program. The HNC is a program without walls that reaches into the homes, schools, churches and other natural gathering places in the community.
This nurse-led, interprofessional, academic primary health care center located directly in the heart of the community it strives to serve. The HNC was established to provide an environment that would maximize nurses’ ability to advance the health of the community.
What is a Nurse Managed Health Center?
Nurse Managed Health Centers (NMHC) are managed by nurses and most are in partnership with low-income underserved communities. They strive to be holistic and interdisciplinary while providing a full range of primary health care services that emphasize health promotion and disease prevention. NMHC are an important component of the safety net health care system that provides care to the truly needy in our society.
