UofL faculty and staff instructions:
If you ELECT medical insurance through the University of Louisville:
1. Complete the Enrollment Packet [pdf]
2. Bring with you when you first visit the facility.
If you DO NOT ELECT medical insurance through the University of Louisville:
1. Complete the Get Healthy Now Waiver Participation Form (PDF)
2. Complete the Enrollment Packet [pdf]
3. Complete both forms and bring with you when you first visit the facility.
