Incentive Program

The University of Louisville would like to reward you for participating in the Get Healthy Now program! The incentive is available for all employees and their spouses (qualifying adults) who are participating in the University of Louisville health plan or employees who waive insurance but choose to pay $20/month for membership in the Get Healthy Now Program.

How it Works

You will be given a $20 per month premium reduction on your health plan costs by completing a health risk assessment (HRA) and actively participating in health coaching (if you are identified as moderate or high risk based on the HRA).

Every employee that completes the HRA will receive an e-mail (welcome / introductory invitation from Health Fitness Corporation, HFC) to call and learn about the health management program and resources offered.  This does not require immediate action.

In addition to the e-mail, all participants will receive a phone call from a health advisor to discuss the results of their personal health assessment.

Additionally, the health advisor will share program compliance guidelines, including health coaching requirements (which will be required for some employees who identify health risk on the HRA that need to be modified).

What is required for program participation?

Annual completion of the on-line health risk assessment (HRA) and regular monthly communication with a health coach IF HRA results indicate medium-to-high health risk.
