 UofL’s AWARD WINNING HEALTH MANAGEMENT PROGRAM

In 2012, all employees/retirees and their spouse/QA have the opportunity to participate in Get Healthy Now. Please read and follow the instructions below to ensure you elect this valuable benefit during this year's Open Enrollment event.

If you HAVE health insurance through the University of Louisville:

1. Take the online Health Risk Assessment (HRA) annually
2. Complete Health Advisory Call to review results and determine additional requirements for program participation.
3. Participate in Empowered Health Coaching program if HRA indicates medium-high health risk(s).

1. $40 monthly premium incentive
2. Additional incentives can be earned based on program participation in the on-site initiatives.
3. Health Excellence (Life well lived!)

If you do not have health insurance through the University of Louisville:

1. Complete the Get Healthy Now Waiver Participation Form
2. Complete and submit to Get Healthy Now via email or fax.

1. Access to all on-site initiatives for no monthly fee.
2. Incentives can be earned based on program participation (Healthy Recipe Cook, GHN Lunch Box, etc.) in the on-site initiatives.

Get Healthy Now on-site initiative offers FREE biometric screening

• Provides a comprehensive and accurate “snapshot” of your health profile: Body Mass Index (BMI), Cholesterol, Glucose, Blood Pressure, Waist to Hip Ratio
• Use screening results to complete the online HRA:
  MANDATORY: Annual completion of the HRA is REQUIRED for participation in Get Healthy Now.
  OPTIONAL: The biometric screening is optional.

• Retirees/spouses/QA (Pre 65): Oct. 3- October 31, 2011
  
• Employees/spouse/QA: Oct. 3- October 31, 2011

REMINDER:  All benefit elections need to be made during the 2012 Open Enrollment: October 17-31, 2011.

• Annual completion of the health risk assessment is required for participation in the health management program, Get Healthy Now.
