Diabetes Management Program
The University of Louisville is committed to improving your diabetes wellness. As a part of your total benefits package, the University is offering a new program to empower you to take charge of your diabetes through medication therapy management with a pharmacist, and diabetes education with a Certified Diabetes Educator. In fact, your participation in this program qualifies you to receive your diabetes, hypertension, and cholesterol medications for FREE!
UofL Health Care Diabetes Management
We are bombarded with seemingly conflicting health messages every day. Making decisions about the best choices in food, portion size, exercise, and rest can be confusing at best; and when you have diabetes, those decisions can seem even more complex.
As a UofL employee, however, you have the opportunity to receive personalized health education and your diabetic medicine and supplies for free! The University of Louisville and UofL Health Care are proud to sponsor the UofL Health Care Diabetes Management Program for UofL employees.
Here are the basics:
- To enroll, call 502-562-4009 to make a one-hour appointment for preliminary blood work and diabetes management assessment.
- Once you are enrolled as an active member of the program, all hypertension, cholesterol, oral and injectable diabetes-related medications, insulin, and supplies are provided at no cost to you.
- Diabetes self-management classes taught by a Certified Diabetes Educator will help you learn to avoid complications and live the healthiest life possible!
