Potential conflicts of interest may be discussed with or reported to:

1.  Your direct supervisor,

2.  The COI Office at 852-7612 or at COIOFF@louisville.edu, or

3.   The Annual Attestation and Disclosure Form (ADF) which will be implemented after an automated submission system is operational.

If for some reason you are uncomfortable in using the above noted methods for reporting a conflict of interest issue, the Compliance Helpline is available as an alternative method of reporting your concern.
