Chronic Condition Support Program

*NEW Updated Requirements for the Chronic Condition Support Program for 2026

The Chronic Condition Support (CCS) program engagement requirements have recently been updated to help ensure members receive consistent support for their health and continue to qualify for the prescription discount. To maintain your prescription discount, you must complete one activity below, once per month.

  • Participate in at least one coaching interaction with your Health Advocate nurse.
  • Complete one full Workshop (6‑day minimum)
  • Enroll in a Pathway and complete one weekly module during the month.

Members who do not complete one activity mentioned above, once per month, for two consecutive months, may see their discount temporarily paused until activity resumes. This update ensures continued support and keeps the program impactful and fair for all.
