Initial Note Taking Evaluation

Hello everyone! It's that time of year again for you to give us an evaluation of your note-taking experience. Not only does your response assist us in providing and maintaining quality accommodation services, but it also gives us an indication of your student's participation up to this point in the semester. It is your responsibility to complete and return this service evaluation.

Has the student contacted you?


If so, how?
Does the student attend class regularly?


Have you made personal contact with the student in or outside of class?


Have you spoken to the student about his or her needs?


If you use a mailbox, does the student pick up the notes promptly?



Have any problems occurred with the student that could not be worked out?


Do you need to speak to the Note Taker Coordinator?
