Final Note Taking Evaluation

Thank you again for your service! It's that time of year for you to give us a final evaluation of your note taking experience this semester. Not only does your response assist us in providing and maintaining quality accommodation services, but it also gives us an indication of your student’s participation throughout the semester. If you took notes in multiple classes or for multiple students, please fill out a separate evaluation for each student in each class.

Did you serve as a note taker for the student you were assigned until the end of the semester?


Did the student attend class regularly?


Did you speak to the student about his or her needs?


If you used a mailbox, did the student pick up the notes promptly?



Did any problems occur with the student that could not be worked out?

Do you need to speak to the Note Taker Coordinator?


Would you like a certificate showing the service hours you have earned as a note taker?
