Criminal Background Check Policies and Procedures
Policy
In furtherance of its purpose, the ULSD requires the following upon the offer of conditional admission for all its programs:
Application
- A criminal background check (CBC) will be administered through either: Associated American Dental Schools Application Service (AADSAS), Dental Hygiene Centralized Application Service (DHCAS), Centralized Application for Advanced Placement for International Dentists (CAAPID), Postdoctoral Application Support Service (PASS) on all applicants as described in this policy.
- Upon initial, conditional acceptance to ULSD, the selected company (Vendor) will send an email notification to the applicant to obtain consent for the CBC to be conducted. Applicants must consent, submit to, and satisfactorily complete a CBC as a condition of matriculation into ULSD.
- Once consent has been provided, the Vendor will conduct a CBC and produce a report.
- When the report is complete, the Vendor will send an email to the applicant requesting that he/she/they review the report prior to its distribution. Upon receiving this email, the applicant will have ten calendar days from the date the email is sent to review their report prior to it being made available to ULSD. If they do not review this report or if there is no objection to the report, the applicant may release the results immediately or the report will automatically be distributed after this period elapses.
- Once distributed, the ULSD Criminal Background Check Review Committee will review and make a determination as to satisfactory completion. The ULSD CBC Review Committee shall include the following:
- International criminal records searches are generally performed by facilitating the applicant in obtaining an official Police Clearance or Police Certificate from the desired country. In those countries that have established third-party criminal records search facilities—including Canada and Great Britain—searches will be obtained directly through those channels.
- Chair of the Admissions Committee,
- Assistant Director for Student Affairs,
- Assistant Director for Admissions,
- Assistant Dean for Admissions and Students Affairs (advisory only),
- University Counsel representative (advisory only).
- The CBC Review Committee shall base its review of the report on whether the results bear directly on the applicant’s ability to matriculate and train in the dental profession. For purposes of this determination, the CBC Review Committee may be guided by the applicable laws and regulations of the Kentucky Dental Practice Act.
- An applicant may submit a written explanation of the result(s) yielded in the CBC report for consideration by the CBC Review Committee. The CBC Review Committee reserves the right to request additional information from the applicant as necessary.
Omission of required information, including the failure to provide the requisite consent, or submitting false or misleading information by the individual in connection with the CBC may result in the ULSD’s withdrawal of the conditional acceptance. False or misleading information discovered after matriculation could result in dismissal.
All expenses associated with the CBC are the responsibility of the applicant.
Confidentiality of CBC files will be maintained to the extent permitted by applicable law.
Additional CBC report(s) may be required during a student’s enrollment in the University of Louisville School of Dentistry.
