University of Louisville Firewall Information Request Form
The implementation of a firewall will add needed security and will complement other enterprise security measures such as virus protection.
This questionnaire is to help the University provide an Internet perimeter Firewall to protect from undesirable Internet traffic. In order to facilitate this change with as little disruption as possible to our end-users, we need the below form completed to identify all servers and services being used.
Because of the nature of firewalls, any unidentified servers and services may stop working when the internet permitier firewall is implemented.
