Copyright Violation Form
Failure to follow any and all instructions will delay the return of your internet access. It is in your best interest to read and understand everything below.
Procedure
You have arrived at this page because you have been flagged for a Copyright Violation. Your access has been disabled. You must complete and submit the form at the bottom of this page in order for your access to be restored. Once this form has been completed and submitted, and this is your first offense, then your seven (business) day ban period will begin. Failure to complete this form will result in your access remaining banned until you complete and submit this form.
An email was also sent to your university email account. The email has the same instructions listed on this page, as well as the actual notice we received from the copyright protection agencies detailing the material in question.
If this is your second or third offense, you must schedule a date to meet with the Security Administrator using the form below. Once you have chosen a time, your appointment is not confirmed until you receive a confirmation email telling you where to go. Please schedule your meeting at least 48 hours in advance.
If this is your first offense, then meeting with the Security Administrator is not required, therefore you do not need to schedule a date and time.
Please remove any illegally obtained files from your web/shared site. Your computer has been banned from accessing the Internet and most university resources. Do not move to another port as you will still be unable to access the Internet.
You have the right to appeal this issue with the Housing Director at sdstat01@louisville.edu.

Copyright Violation is not only a federal offense but it also violates your computer account usage agreement with the university. Please review the University's policies.
