Online Graduate Application for Admission

Note: To enroll in Graduate Studies, you must have earned a baccalaureate/undergraduate degree from a regionally accredited college or university. 

Students applying to the following programs must first complete a SOPHAS application and then complete an application to the Graduate School:

Master in Public Health
Master of Science in Health Administration (SOPHAS application is NOT required for the online MSHA program)

To apply to any of the MBA programs or the Masters of Accountancy, refer to the College of Business website.

Students applying to the PhD, DNP or MEPN programs in Nursing, must first complete a Nursing’s Centralized Application Service application (NursingCAS) and then complete an application to the Graduate School.

Application Fee Waiver For: who are currently serving on Active-Duty, in the National Guard, or in the Reserves in; the U.S. Army, the U.S. Navy, the U.S. Air Force, the U.S. Marine Corps or the U.S. Coast Guard

For all other programs, complete the Graduate Application for Admission following the steps below:

  1. Read the Application Directions
  2. Create an Account (This account is separate from any ULink/UofL account.)
  3. Complete the Application

Electronic versions of the paper Letter of Evaluation form is available for download below. Please do not use this form if you are using the online option with your application.
