Remote Access Instructions

 General Information

Please use the application form below to submit a request to allow remote access to University of Louisville School of Dentistry's patient care management system(s) using your personal computer. Please review the requirements below:

Don’t forget to complete every numbered point, or you will not be granted remote access.

1) You must read the ULSD Remote Access Policy located here. You will be asked to sign the agreement form in step 5 (below) indicating that you have read and understand the responsibilities and requirements that come with remote access.

Failure to strictly follow the policy is a HIPAA violation and will result in loss of privileges.

2) You are required to change your computer name to the appropriate format listed below.

VERY IMPORTANT: The naming convention must be followed for access to be granted.

Note the following:

·         Your student/employee ID is the 7-digit number on your UofL ID card, not your alphanumeric ULink ID.

·         You are only able to have remote access on one computer.

3) UofL Two-Factor Authentication application (DUO Mobile) is required on your mobile device. Click Here to sign up.

4) Duo Desktop is also required to be installed on your PC or laptop, which ensures your device is up-to-date and compliant. To access DUO Desktop for Windows OS click here and for MAC OS click here.

5) Submit your signed Remote Access Security Agreement form to
di@louisville.edu (click here to download).
Your remote access request will not be processed without a signed Security Agreement.

6) Complete the form below. Once submitted, a ticket will be generated on your behalf, and your request will be processed.

You will receive an email from ULSDHELP with further instructions.


To provide Dental Informatics sufficient time to process submissions, please place requests at least one week prior to your need for remote access.
