Accelerated MPH Petition Form

The accelerated curriculum allows students to take up to 15 graduate credit hours of the MPH core course work during their undergraduate studies. The 15 credit hours will count toward the completion of BA and MPH Degrees. Any student meeting the criteria and interested in the Accelerated MPH program is required to submit a petition form and a letter of recommendation. 

Admission criteria:

-- Current BA in Public Health
-- Completed 75 credit hours by Aug 2020
-- Minimum GPA of 3.25

Drop-Add Form

The late drop/add form is a manual way for students to drop/add a course past the posted deadline. The signed late drop/add form is used for this purpose.
• The deadline to add a course is the last day of the first week of classes. After that date, students must have special permission to enroll via the signed late drop/add form.
• The deadline to withdraw from a course changes each semesters. Students should check the academic calendar for the specific deadline (https://louisville.edu/calendars/academic/undergrad-grad). After this date, students must have special permission to withdraw from a course via the signed late drop/add form.
• The deadline to change a course from credit to audit is the same as the last day to add a class for the term/semester (see above). The signed late drop/add form is used for this purpose.
The completed/signed late drop/add form should be sent to the Office of Student Services for Dean's signature and processing.

Petition to Repeat Form – Graduate Students Only

The petition to repeat form allows students to repeat a course that they earned a C or below in. This form should be submitted to your department and the department will complete a supplemental form, which will be provided to you, that determines the required grades need to graduate with at least a 3.0 from your program.

Re-enrollment Form

The request to re-enroll form allows graduate students who have not attended the university in consecutive semesters (excluding the summer semester) to re-activate their enrollment account with the University of Louisville. Students who have not registered for more than two years will be required to re-apply to the university.

Request for Degree Awarded Letter

The Office of Student Services will provide a letter indicating that you have completed all degree requirements, however, your degree has not yet been posted to your official transcripts. 

(It can take up to several weeks for your degree to be posted to your transcript)

Request for Travel Letter for International Students

The Office of Student Services will provide a letter for international students traveling outside of the United States indicating that you are a current student and will be registered for courses in the upcoming semester.

Request to Withdraw from Graduate Program

Please complete this form if you are withdrawing from your program.

Travel Scholarship Form

The Office of Student Services will provide a letter for international students traveling outside of the United States indicating that you are a current student and will be registered for courses in the upcoming semester.

Variance Request Form

The variance request form is required when there is a variance in the program requirements:
• Admission
• GPA
• More than 2 C's
• Leave of Absence

Please complete student section and then submit to department. The department will obtain signature and submit to the Office of Student Services for Dean's signature and processing.
