Tips on Filling Out Your Kentucky LPL

You must have completed 60 credit hours before you can apply for a Kentucky Limited Practice License. If you have not completed 60 hours, you may not apply.

Page 1: This is for information only; you do not have to turn this in. Please disregard the instructions about obtaining a certified check for $25. You should pay your fee online at https://payments.lexisnexis.com/st/kentucky/baradmission (Select Student Intern. $25).

Page 2: This page will be returned to you when completed as your LPL.

  • Please sign Item 1 on the signature line. Do not type your name on the signature line. See this link for more about signing this document.
  • Item 2 is for your Faculty Director to sign. This will vary depending on your placement. Your faculty director will either be: (Externship 989 or 991), (Ackerson Clinic), (Trager Clinic), (Entrepreneurship Clinic), or (summer internships). Contact one of these individuals to obtain a signature for Item 2.
  • Item 3 is for Dean Sweeny to sign. You may email or bring the signature page to and she will handle Dean Sweeny's signature for you.
  • Item 4 is for your site supervisor to site. This will vary depending on your placement. Your site supervisor will be the person overseeing your work. It may be Heend Sheth (Ackerson Clinic), Lee Shaw (Trager Clinic), Will Metcalf (Entrepreneurship Clinic), or a member of the public. You are responsible for obtaining this signature.
  • Items 5 and 6 are to be left blank - they will be completed by the state.

Page 3 (Item 2 Applicant Information): Father's name and address/Mother's name and address. Please complete these areas using your parents or guardians' names and addresses. If you do not have current contact with or knowledge of a birth parent, you may leave that space blank or substitute the name of a step-parent or guardian whom you would consider in that place. If you have two parents of the same gender, feel free to amend the form and complete as necessary.

Page 5 (Item 5 Character References): If you do not know two practicing attorneys, several faculty members have active law licenses including Professor Canon, Professor Sheth, and Professor Dunman.

Pages 10, 11, and 12: Do not complete these pages until you are in the presence of a notary. Kim Sanders will notarize these for free if you contact her.

You will recieve a copy of your signature sheet from the OBA. If you do not recieve one within a month, contact info@kyoba.org for updates. The law school should also receive a copy of your signature sheet and will email you when recieved. If you recieve your copy first, please forward it to Kim Sanders.
