Passport Place

Welcome to the Passport Place at the University of Louisville! The Passport Place is authorized to accept Passport applications from eligible U.S. citizens on behalf of the Department of State.

Location: The Passport Place is located on the lower level of Jouett Hall (Room LL02) on the campus of the University of Louisville. The University of Louisville is located at 2301 South Third Street, Louisville, KY 40292.

Hours of Operation: Monday – Friday 9:00 am – 4:00 pm,
Closed on University Holidays.

No appointment is needed at this time. The Passport Place is open for walk-in appointments.

Timeline for Applying for a U.S. Passport

Planning to travel? Apply early! It is advisable to apply at least 4-6 months before your planned travel.

Processing times for routine and expedited service are as follows:

  • Routine Service - 7-10 weeks beginning when the application is received by the US Department of State
  • Expedited Service - 4-6 weeks beginning when the application is received by the US Department of State. Expedited services cost an additional $60 per application.
  • It may take up to 2 weeks from the day you submit your application until your status is "In Process." During these 2 weeks, your application is delivered to the mail facility, your payment is processed, and your application is entered into the system. Even if your passport status indicates, "Not Available," your application and supporting documents are safely on their way to the US Department of State.   • Optional USPS Express postage can be added to any service for 1-2 day delivery one/both ways.

Customers with life-or-death emergencies should make an in-person appointment at a Passport Agency or Center. More information about appointments is available.

What to bring to your appointment
  • On the day of your appointment, please come prepared with the necessary forms completed and filled out in black ink.
  • Bring proof of citizenship (birth certificate or naturalization certificate). You must present the original and one black and white photocopy. If you do not have a photocopy, copies can be made at the Passport Office for 25 cents per page.
  • Bring identification (driver’s license, state ID card, or military ID card). You must present the original and one photocopy with the front and back of the document on the same page.
  • We do not provide photo services in our office. You must bring an acceptable/valid passport photo when you apply for a passport.
  • If you are applying for a passport for a child under the age of 16 please visit travel.state.gov and review the requirements carefully. The child must be present for the appointment. Both parents are typically required to be at the appointment.
  • Two separate payments are required to the U.S. Department of State and the University of Louisville:
    • Check or Money Order accepted for Passport Fee(s) payable to the U.S. Department of State for the specific services being requested (amount is TBD based on the services requested)
Applying for an ADULT Passport

Please note: Renewals of passports do not require a visit to a Passport Acceptance Facility. Renewals can be mailed to the National Passport Processing Center. See the instructions on Form DS-82 (PDF).

Important Note: Do NOT submit any passport application information to our office via email. We cannot accept applications via email. You must bring your application to the Passport Place in person.

  2. Submit completed form DS-11 In Person – Jouett Hall, Lower Level, Room LL02
  3. Provide evidence of U.S. Citizenship - certified birth certificate OR naturalization certificate (Please bring original certificate and one copy)
  4. Present identification - photo ID (driver's license or military ID)
  5. Submit a photocopy of the identification document(s)
  6. Pay the applicable fees—two payments as prescribed below—NO CASH ACCEPTED (see the fee section below)
  7. Provide one passport photo
Applying for a CHILD Passport

To apply for a United States Passport, you will need to follow these steps:

  1. Complete Form DS-11: Application For A U.S. Passport (PDF)
  2. Submit completed form DS-11 In Person – Jouett Hall, Lower Level, Room LL02
  3. BOTH parents need to be present (If one parent has legal and physical custody you must provide original court order)
  4. Provide evidence of U.S. Citizenship - certified birth certificate OR naturalization certificate (Please bring original certificate and one copy)
  5. If child has previous passport, please bring BOTH passport and original birth certificate.
  6. Present parental identification - photo ID (driver's license or military ID)
  7. Submit a photocopy of the identification document(s)
  8. Pay the applicable fees—two payments as prescribed below—NO CASH ACCEPTED (see the fee section below)
  9. Provide one passport photo
    Forms

    Form DS-11 (PDF):

    • Applying for a U.S. passport book or card for the first time
    • Current U.S. passport is valid but has been lost, stolen or damaged
    • Previous U.S. passport was issued more than 15 years ago

    Form DS-64 (PDF):

    • Used to report the loss or theft of a valid U.S. passport book or card
    • Submitted with Form DS-11 and supporting documents to replace a previously lost or stolen passport
    • Used alone to report the loss or theft of a passport even if you do not want a replacement passport

    Form DS-3053 (PDF):

    • Minors under age 16
    • Completed by the non-appearing parent
    • Must be notarized within the last 90 days, and must be accompanied by a copy of the non-appearing parent's photo ID
    • Submitted with Form DS-11

    Form DS-82 (PDF):

    • To renew an existing, 10-year passport
    • To replace a limited-validity passport that is more than 1 year old
    • Only use if current passport was issued less than 15 years ago
    • Only use if name on the application and the previous passport are the exact same (unless you are submitting a legal name change document or marriage certificate)
    • Only if the passport is undamaged and can be submitted by mail with Form DS-82
    • In order to protect the contents of your mailing from the elements throughout the delivery process, it is strongly recommended that you mail your passport application and personal documents in an envelope large enough to fit the application without folding. Please do not use a standard letter-size #10 (4 1/8" x 9 1/2") envelope.

    Form DS-5504 (PDF):

    • To request a name change
    • To correct data errors (typos)
    • To replace limited-validity passports (if eligible)
    • You may submit Form DS-5504 by mail
    Fees

    The Passport Place does not accept cash, credit or debit cards, or electronic payment of any kind for payment of fees.

    You will need to bring two separate payments with you:

    1. Check or Money Order accepted for Passport Fee(s) payable to the U.S. Department of State for the specific services being requested (amount is TBD based on the services requested - see below)
    2. Check or Money Order accepted for Execution Fee (amount is $35 per application processed) payable to the University of Louisville.

    Note: A $20 fee will be assessed for all returned checks.

    Optional Fees (Paid to the U.S. Department of State)

    Expedite Fee Paid per application, in addition to required fees. Provides faster processing than routine service. $60
    1-2 Day Delivery Paid per application for 1-2 day delivery of an issued passport book from the Deparment of State to the customer. Only applies to mailing addresses within the United States. Not valid for passport cards. $18.32
    File Search Fee A file search is necessary when an applicant is unable to present evidence of U.S. citizenship or verification of a previously issued U.S. Passport or Consular Report of Birth Abroad. Applicant must submit Form DS-11 (PDF) and a writen request for a file search. $150
    Important Notices

    Kentucky residents will need either a valid US Passport or the Voluntary Travel ID (REAL ID-compliant driver's license) for domestic air travel effective May 3, 2023. You may also find more helpful specifics on drive.ky.gov.

    COVID-19 Updates The U.S. Department of State is advising extreme caution with international travel, depending on infection rates. Effective, May 4, 2022, masking is only required in clinical areas on campus. In all other areas, masks are optional regardless of vaccination status.
