Admissions Requirements
In order to submit an application to one of the School of Public Health and Information Sciences (SPHIS) degree programs, you must apply to the University of Louisville Graduate School. Admission requirements for each SPHIS degree program are listed below.

| Certificate in Clinical Investigation Sciences | Fall semester enrollment – Completed applications and all required application materials are due by August 1. International students are encouraged to apply no later than 120 days prior to the start of the fall semester. Spring semester enrollment – Accepted with Program Director approval. Summer semester enrollment – Applications are not accepted. |

Those received after December 31 will be considered based on availability of space in the program. Spring semester enrollment - Applications are not accepted.
