Welcome to Phase II of the Type 2 Diabetes and Insulin questionnaire study!

We appreciate your help with the hand written questionnaire that you completed for Phase I. Phase II consists of web-based follow-up surveys at 6 weeks and 3 months from your completion of the Phase I paper pencil questionnaire packet that was given to you at the time of your blood draw at Joslin. These Phase II online questionnaires will closely follow the content of the original Phase I questionnaire packet. All website questionnaires are confidential and linked to the participant ID that was developed specifically for this study. This is the ID number that was given to you at the time of the Phase II reminder phone call or email.



If this is your first Phase II survey (i.e., 6 weeks since you completed Phase I), you will be entered into the phase II part 1 lottery once your online survey has been submitted and we will be in touch with you in approximately 1 month to remind you about the second and final follow up. If this is your second Phase II survey (i.e., 3 months since you completed Phase I), you will be entered into the phase II part 2 lottery once your online survey has been submitted. If you have completed all 3 surveys (hand written Phase I and both Phase II 6-week and 3-month web follow-ups) you will also be entered into comprehensive study completion lottery.

Troubleshooting


If you are having difficulty with the online survey or have forgotten your ID number, please leave a message for research staff at (502) 852-3629(Health Behavior Change Research Program at UofL). In your message, please include your name, your phone number, and whether or not it is okay to leave a message at that number. Please also indicate that you are a participant in the Type 2 Diabetes and Insulin Study. This is a password-protected mailbox. A research staff member will return your call and help you with the problem.
